Dealing with insurance companies can be challenging. Please review this information to make sure you are getting the maximum benefit from your insurance carrier for reimbursement of therapy services.
Concorde Kids will verify coverage for all new patients , as well as those patients changing policies or coverage. Please understand that ultimate responsibility for payment of services lies with the responsible party, and we will not initiate services unless the responsible party for payment of bills authorizes services..
We will not accept responsibility for incorrect or incomplete information received at the time of verification.
If a referral is needed from your primary care physician to initiate therapy services, please be sure you have it in hand prior your appointment.
Our services are available under direct access in Ohio, but most insurance carriers require a physician's prescription for reimbursement. You should take the step to get a prescription from the referring physician for the services you are receiving along with a diagnosis. The physician can fax it to us at 330-478-1763, or you may bring it with you to your first visit.
When you have a limitation on visits, it is your responsibility to keep track of how many you have had for the year. If you exceed the allowed visits for the year, you will be responsible for payment of denied therapy services.
Please notify us immediately when you change insurance companies. Bring your insurance card to every visit. If you do not notify us of a change in insurance, you will be responsible for any non-covered charges.
The Medicaid program now covers Occupational Therapy through our facility - ask us for details.
If you have two insurance companies and your primary carrier has a co-pay, you must pay the co-pay at the time of service.
Keep all Explanations Of Benefits (EOB) from your insurance carrier. Review them carefully.
Although we are willing to bill all insurance companies, we are not networked for all of them. Please check with our Office Staff to see if we are panelled your insurance company. You are responsible for all charges not covered by the insurance company.
